What data fields are required to create a UIN?

The following fields are always required to create a UIN:

  • Last Name
  • Sex/Gender

One or both of these fields are also required:

  • Date of Birth
  • Social Security Number

Note that both Date of Birth and SSN are strongly recommended to increase the uniqueness of the UIN data, which results in fewer errors and a more reliable UIN assignment.

Additionally, the full name of the employee is strongly recommended:

  • First Name
  • Middle Initial
  • Last Name
  • Suffix (Jr, Sr, III, etc.)
